Paying for Care
We accept motor vehicle/auto accident claims, worker comp claims and most major medical insurance companies. We also offer a prompt-pay discount for those paying for care without insurance and a Wellness Membership plan for patients looking for regular wellness treatments at a discounted rate.
We accept Visa, MasterCard, American Express, in-state checks with a valid ID or cash for co-pays, co-insurance, deductibles or prompt pay discount appointments. Making Your Appointment
To make your first appointment, please call our office at 503-493-9398 and choose option "1". Tell our front desk coordinator that you are a new patient and would like to schedule an appointment. You may also email the front desk at npwc@northportlandwellness.com.

Your information
When you call us for your initial appointment, our staff will ask you for the following information:
- Your first and last name
- Your phone number
- Your email address
- Your date of birth
- Debit or credit card to hold your appointment
- Whether or not we’ll be billing insurance for you
- Your insurance company name, ID#, and group # if using insurance
- If you are not the primary on the insurance, please provide the name and DOB of this individual
If you have been involved in Auto or Work-related Accident, you will also need to provide the following:
- The insurance company name
- Your claim number
- The date of the accident
- The state in which the accident occurred
- The name and phone number of the adjuster or claim representative with whom you’ve been working

Scheduling
The front desk staff will then work with you to find a time for your initial appointment. We offer reminder emails two days in advance of appointments as a courtesy to our patients. If you need to alter this appointment time, please note we require at least 24 hours notice.
Paperwork
All new patients are asked to complete our online New Patient Paperwork. You will receive a link to the paperwork via email when your first appointment is scheduled. This paperwork will need to be completed prior to your first appointment and can be completed using any computer, Chromebook, tablet or cell phone from the comfort of your home.
Items to Bring
Please bring the following items to your first appointment:
- Valid ID
- Your current insurance card
- Forms of payment: We accept cash, check, and Visa/MC
